Octreotide for Treatment of Postoperative Chylothorax after Cardiac Operation

Abstract

|
Chylothorax may occur spontaneously or as a complication of thoracic surgery. Chylothorax especially after pediatric cardiac surgery is associated with significant morbidity and prolonged hospitalization. Management may include cessation of enteral feeding, repeated aspiration, chest tube drainage, total parenteral nutrition, and introduction of medium chain triglyceride(MCT) diet as the effusion resolves. We presented a successful case of octreotide(a synthetic somatostatin analogue) infusion(2-4 ug/kg/hr) with medium chain triglyceride diet which hastened the resolution of postoperative chylothorax in a pediatric cardiac surgical patient. Resolution of the chylothorax was seen within 8 days and did not recur after the patient was back on normal diet.
